Sukuna had monitored Megumi ever since their initial confrontation at the juvenile detention center. The King of Curses was never interested in Megumi's fragile human vessel for prestige; he coveted the versatility of the Ten Shadows technique and saw Megumi as an escape hatch from Yuji Itadori's unbreakable biological cage. In Chapter 212, Sukuna ripped off his own pinky finger, concentrated his soul into the cursed object, and force-fed it to Megumi while the boy stood paralyzed in horror.
Possessing Megumi proved fundamentally different from cohabiting with Yuji. While Yuji acted as a natural prison capable of suppressing Sukuna at will, Megumi lacked that innate physical anomaly.
